(1) Vendor needs to provide medical director service for the position of medical director for the soldier's home to the government authority located in Topeka, KS.
- To provide general medical consultation, treatment and services for the residents of KSH in accordance with standards set by the VA, CMS and the department for aging and disability services (KDADS) for residents of long-term care.
- Conduct initial evaluations within 48-72 hours of each resident upon admission to the facility, unless the resident has a local primary physician. The evaluation shall include: a report on current medical findings, diagnosis, orders for immediate care of the resident, and a medical history.
- Is responsible for reviewing the resident's total program of care, including medications and treatments, at each visit. Shall conduct rounds as medical conditions warrant and utilize facility's clinic when necessary.
- Is responsible for writing physician progress notes when there are significant changes in the resident or when laboratory values or treatment needs change, and periodically if the status is stable.
- Supervises the medical care of each resident.
- Serves and actively participates on facility committees such as quality assurance, pharmacy, infection control and internal reviews for sentinel events. Actively participates in the credentialing and privileging process.
- Supervises and oversees the practices of any physician's assistant(s) and nurse practitioner(s) working at the soldiers home.
- Maintain on file with the Director of Nursing a current copy of his and her license to practice medicine in the state and a current copy of his and her controlled substance registration certificate and a current copy of liability insurance certificate.
- Maintain general and professional liability policies to ensure them, their employees, and any other person providing services. The minimum amount will be $1,000,000 per occurrence and $3,000,000 in the aggregate. The Contractor shall give at least 15 days' notice of any cancellation or lapse in coverage.
- Shall be familiar and comply with all organizational Policies and Procedures. Submit a monthly invoice for payment, based on the agreed upon hourly rate. Invoices shall include number of residents seen, dates of those examinations, and duration of each visit.
- Shall submit monthly time sheet to comply with Payroll Based Journaling requirements established by CMS regulation.
- Contractor shall provide or arrange for provision of physician services 24 hours a day, seven days per week and physician back up for emergencies, illnesses, or other time off needs plus comply with credentialing criteria and requirements for back up physicians.
- Communicate with the Medical Director regularly concerning all relevant medical information and quality concerns.
- Provide adequate space and equipment at the facility for resident exams, evaluations and care.
(2) All the questions must be submitted no later than July 05, 2023
