Government Authority located in Topeka, Kansas; USA based organization looking for expert vendor for registered dietitian services.
[A] Budget: Looking for Proposals
[B] Scope of Service:
(1) Vendor needs to provide dietitian and nutritional assessment services to the government authority located in Topeka, KS.
- Assist the Hospital in providing dietitian services to patients and residents including assessment, follow-up and education specifically:
a. Complete at least two nutritional assessments per week documenting on the nutritional assessment form. Written orders should be first priority and patients and residents with consultation with medical concerns based on diagnosis should be second priority and be decided upon with consultation with the medical director.
b. Monitor patient food and nutrient intake and evaluate effectiveness of nutritional care plan.
c. Participate in patient care team meetings or conferences.
d. Monitor canteen orders weekly for contraindications with diets, allergies, medical conditions, etc. (e.g. diabetic patient ordering a candy bar, etc.). Notify the medical director’s office and unit physician if any are found.
e. Conduct and document test tray reviews weekly.
f. Conduct a mock survey of kitchens as needed and submit a report to the quality management and food service directors.
g. Provide training to the staff in aspects of nutrition and disease management.
h. Involve patient in setting realistic goals pertaining to nutrition and disease.
i. Document nutritional assessments and interventions in the individual electronic medical record to assure continuity of care. To work with the medical staff in proper diet selection, supplements, snacks, etc.
j. Interpret and incorporate new scientific knowledge into practice.
- Provide an itemized invoice every two weeks setting forth the services rendered, and time spent providing service to the CFO for approval and then to financial services for payment.
- Provide dietary consultation services no more than two to three 8-hour days per week as requested or patient and resident needs indicate.
(2) All questions must be submitted no later than June 19, 2019.
