[B] Scope of Service:
(1) Vendor needs to provide cloud-based, healthcare hazard vulnerability assessment tool targeting the needs of health and medical providers that has the ability to create, assign and control login id’s and permissions, to enter data in pre-configured customizable forms, allow users to instantly view and analyze data in graphs and charts and ability to upload geographical and census data.
(2) All the questions must be submitted no later than August 17, 2017.
(3) A contract term will be for one year.

Onshore (USA Only)
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office premises.
