[B] Scope of Service:
Vendor needs to provide coordinated system of donated medical care for uninsured, low-income citizens in county to the government authority located in Wichita, KS.
- Community Services Block Grant funds to be used within an existing coordinated system of health care with access to leveraged or donated physician and dental services that address immediate medical needs.
- Statement should include a detailed description of the existing coordinated healthcare system access through leveraged or donated physician and dental services; including the number of participating physicians, hospital systems, dentists, pharmacies and other allied health care services, such as physical therapy and hospice care; and the experience detail, eligibility process, number of low-income patients served and all other pertinent data.
- Program Objective is to utilize community services block grant funds within an existing coordinated system of healthcare access through leveraged or donated physician and dental services for uninsured, low-income adult (age 18+) individuals who reside in county.
[C] Eligibility:
- Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location
